Composite License (Business / Industrial) City Council Shah Alam


Introduction


This Composite License Application is a business/industrial license that covers all areas under the supervision of the Shah Alam City Council. This Composite License is a combination of premises license and billboard license.​​


How to Apply


Forms can be found at the Main Counter on the G floor, the Licensing Department Counter on the 13th floor, Sungai Buloh Branch Office, Kota Kemuning Branch Office and Setia Alam Branch Office. (tingkat 13, Pejabat Cawangan Sungai Buloh,Pejabat Cawangan Kota Kemuning dan Pejabat Cawangan Setia Alam)


The completed form must be accompanied by four (4) copies of each required document.​​​​​​​​

The completed License Application Form must be submitted at the Licensing Department Counter on the 13th floor.


Composite License Application Process


Steps representing the composite license application process:


  1. Accept Application:


    • Receive the application form and supporting documents.

    • Verify the completeness and accuracy of the submitted information.


  2. Review:


    • Assess the application against the relevant criteria and regulations.

    • Identify any deficiencies or discrepancies.


  3. Complete Application Process:


    • Request additional information or documentation if necessary.

    • Ensure that all required steps are completed.


  4. Submit the Application to the Technical Department:


    • Forward the application to the technical department for evaluation.


  5. Site Inspection by the Technical Department:


    • Conduct a physical inspection of the proposed site to assess its suitability.

    • Verify compliance with technical standards and requirements.


  6. Preparation of Documents for Meetings:


    • Gather relevant documents and information for meetings with the applicant and relevant authorities.


  7. MJMDMPL:


    • The relevant department or committee review and discuss the application.


  8. Passed:


    • If the application is approved, proceed to the next steps.


  9. Update Meeting Result and Decision Letter Preparation:


    • Document the meeting outcome and prepare a decision letter.


  10. Review and Sign the Letter:


  • Review the decision letter for accuracy and completeness.

  • Obtain necessary approvals and signatures.


11. Post/Deliver Letter:


  • Send the decision letter to the applicant.

  1. Form D and Form A (ROB).​

  2. Forms 9 and 49 (ROC) or Form 13 (change of name).

  3. Copy of Identity Card.

  4. Copy of Organization/Association/Club Registration.

  5. Copy of CCC/CF. Copy of Building Permit (If Applicable).

  6. You are requested to refer to the Certificate of Eligibility for Occupying the Building (CF).​​

Application Checklist for Business, Industry and Advertising Licenses in the Administrative Area of the Shah Alam City Council


A. General documents that need to be attached


  1. Completed Composite License Application Form.


  2. One (1) piece of passport size photo


  3. Copy of Identity Card (Citizenship) or copy of International Passport and

    Business Visa (Non-Citizen) company owner/director. A copy is required

    crossed out and written "MBSA matters only".


  4. Copies of documents from the Companies Commission of Malaysia (SSM):-


    Registration of Private Limited Company

    - Copy of Complete Company Information (S17 (Form 9), S14 (Forms 24, 44 & 49),

    S32 (M&A

    - Memorandum And Articles Of Association), Company Profile)


    Business Registration (Enterprise/PLT)

    - Copy of Complete Business Information (Form D, Business Information,

    Owner Information/ Business Profile)


    Registration of Associations or Clubs

    - Copy of the Registration Certificate of the relevant Association, Organization or Club.


  5. A copy of the latest Assessment Tax payment bill and receipt for the business premises

    requested.


  6. Copy of the Premises Rental Agreement (the latest one) or Sale and Purchase Agreement Buildings and Title Grants (For Private Land) subject to stamp duty.


  7. Copy of Certificate of Eligibility to Occupy the Building (CF) or Certificate of Completion and Compliance (Form F) or Title Grant (For Buildings Without CF).


  8. Pictures of the Current Premises (Front, Back, Inside and Side Views)


  9. Premises Location Plan (if using Google maps the location must be marked

    premise)


  10. Latest Premises Floor Plan (with measurements in square meters)


  11. Copy of Temporary Construction Permit Approval Letter (Partition)/Additional Amendment.

    (If there are additional amendments, if there are no additional amendments please write TB/Not Relevant):


    1) Office store / Shop House

    It is necessary to obtain the approval of a Temporary Construction Permit for a Partition Wall (Partition) and Sengkuap Roof (Awning) using the Equivalent Plan Book

    (Standard Plan). Please deal at the Building Department Counter, LG Floor,

    MBSA guest house


    2) Terrace factory / Twin factory / Single factory

    Need to submit an additional amended plan to the Local Central Division (OSC)

    for partition wall addition (Partition)/ roof addition (Shed)/ addition

    amendment (space) on the front, side and back. Please deal at

    Central Division Counter (OSC), LG Floor, Wisma MBSA.


  12. A copy of the visual sketch of the ad along with measurements in meters and photos of the location installation of billboards and letters of confirmation of the Language and Library Council (DBP). Please contact DBP at 03-21479100/ 9200/ 9300 for information

    further. (Please refer to Section C from No. 3 to 12)


B. Support documents from external agencies that need to be attached if necessary in relation to the license activity applied for:


  1. Activities of Processing/ Storing/ Removing Hazardous Materials/ Simple

    On fire

    Copy of letter of authorization from:-

    i. Department of Environment (DOE) – effluent discharge from premises activities

    i. Malaysian Fire and Rescue Department – ​​Flammable materials, materials

    petroleum/ solvent/ LPG

    iii. Atomic Licensing Board – Radioactive substances


  2. Activities of storing Firearms/ Explosives

    Copies of approval letters from the Ministry of Home Affairs (KDN) and the Royal Police

    Malaysia.


  3. Activities of cyber centers/cyber cafes, second-hand goods and used car industries

    Copy of Royal Malaysian Police review letter.


  4. Activity of selling/ storing liquor

    Copy of Klang/Petaling District and Land Office License.


  5. Video center activity/ store/ distribute/ sell VCD-DVD

    Copy of Approval Letter from the National Film Development Corporation

    Malaysia (FINAS)/ Ministry of Domestic Trade and Affairs Users (KPDNHEP) and Registration with Music Authors Copyright Protection (MACP).


  6. Livestock Activities

    Copy of the approval letter from the Health Department.


  7. Garbage Collection/Disposal Activities

    A copy of the approval/confirmation letter from the Solid Waste Management Department and Public Cleaning, MBSA.


  8. Activities of Nursery / Care Center

    A copy of the latest Social Welfare Department approval letter.


  9. Activities of Kindergarten/ Teaching Center/ Tuition Center

    A copy of the latest Department of Education approval letter.


  10. Pharmaceutical Activities

    A copy of the Pharmacist's Certificate and Pharmacists Poisons License if there are any to sell items categorized as the latest poison.


  11. Activities of Licensed Money Changers

    Copy of latest license from Currency Exchange Control Department, Bank Negara Malaysia


  12. Activities of Licensed Money Lenders/ Community Credit

    Copy of latest license from Licensing and Advisory Services Division, Ministry Housing and Local Government (KPKT) latest.


  13. Travel Agency

    A copy of the latest license from the Ministry of Tourism and Culture Malaysia.


  14. Laundry Activities

    A copy of the approval to install a gas installer from the latest Energy Commission.


  15. Mini Market Activities/ Retail Stores

    Copy of pest control certificate


  16. Copy of the latest Planning Permission (KM) approval letter for Kindergarten/ Nursery/

    Nursery/ Care Center. (Except for special sites/buildings for Kindergartens/Nurseries/Centers Care) up to date.


  17. A copy of the landlord's permission letter if activities are carried out on the land/area

    roomy


  18. Other activities that require confirmation/approval from relevant agencies: ___


C. Documents and general terms of the advertising board license application in front of the premises (signboard)


  1. Completed Composite License Application Form.


  2. 2. A copy of MBSA's Composite License (Business / Industrial).


  3. A copy of the company registration:-


    - ROB (Form D) or

    - ROC (S17 (Form 9), S14 (Forms 24, 44 & 49), S32 (M&A - Memorandum And

    Article Of Association), Company Profile) or

    - Registrar of Societies or

    - Copy of Medical Certificate for doctors or medical practitioners from the Malaysian Medical Council stating the address of the premises applied for (for clinic advertisement).


  4. The Company's activities must be stated in Bahasa Malaysia and in writing size should be 5% larger than the company name and positioned above company name. (Refer to Appendix 1)


  5. Pictures of the billboard installation location according to the specified location plan.


  6. The billboard sketch information must be completed with the following information:


    i. Company activities;

    ii. The name of the company registered with SSM;

    iii. Company address;

    iv. Billboard size;

    v. Word size;

    vi. LIGHTED OR NON-LIGHTED ad type.


  7. Applicants need to obtain confirmation of language usage for advertisement visuals

    from the Language and Literature Council (DBP). Please contact DBP at 03-2147

    9108/9/10 or visit http://dbpsahbahasa.my for more information.


  8. Installed billboards should not obstruct windowed structures and glassy


  9. Need to get comments from the Malaysian Fire and Rescue Department if installing large-sized advertisements (if necessary) and/or closing the window.


  10. Billboards installed must not extend beyond the roof of the building.


  11. Advertisement structure must obtain the approval of a certified Engineer registered.


  12. Please refer to Appendix 1 for examples of business/ industrial billboards and

    Kindergarten / daycare / daycare center signs in residential premises. Only 1 unit

    only allowed.


  13. Please refer to Appendix 2, Neighbor's Permission Letter if there is a double-sided advertisement on the side/edge of building premises (if applicable). Complete the letter with the company stamp concerned neighbors.


  14. In the event of a new advertisement application/ additional advertisement involving activities the following need to submit an approval/registration letter from the following agency:-


    i. Clinic – Copy of Medical Practitioner Certificate for doctor or practitioner

    medical from the Malaysian Medical Council stating the address premises applied for.;

    ii. Tourism Agency – Ministry of Tourism and Culture Malaysia (MOTAC);

    iii. Other activities that require confirmation from relevant agencies.
