1. Conditions of the Building Department
1.1. Any additional partition wall 'Partition Amendment (Breaking the wall) /
Awning additions from the original plan must be submitted directly to the 21st Floor building, Wisma MBSA by an Architect or Painter Registered Plan.
1.2. Activities to be carried out must comply with the conditions of building use
which has been set.
1.3. You are requested to submit an application for an advertisement license separately if you plan to install a billboard on the premises by complying with the conditions of passing the billboard where the company's activities should be bigger than the company name and the advertisement should be in Malay language.
SAFETY
i. The premises must have a water and electricity supply system and perfect drainage, in good and safe condition.
ii. Display "NO SMOKING" and "KEEP CLEAN" signs.
iii. The entire premises in the indoor and outdoor areas should always be clean, in good condition, free from insects and rodents or pets and nuisances.
iv. Toilets and bathrooms should always be good and clean.
v. Sewage drainage should be drained to the system sewerage.
WASTE DISPOSAL
i. The premises must have a perfect and sufficient covered trash can and in clean and good condition.
3. Conditions of Licensing Department (Health Care Center)
3.1. The front wall of the premises and the entrance of the Health Care Center premises
 should consist of consists of light-transparent mirrors and surfaces. The mirror must be free of any stickers, posters, 'tinted', or anything that could obstruct the view.
3.2. The applicant is not allowed to make any partition (partition) either permanent (concrete) or semi-permanent (gypsum board). Premise should be open including no curtains.
3.3. Female employees are only allowed to massage female customers and employees
man massaging a male customer.
3.4. It is not allowed to place a bed or mattress. For reflexology massages only allowed to park massage chairs that have been approved by the Council.
3.5. The design of the treatment place should follow an open concept and must have perfect light and enough ventilation.
3.6. The licensee is prohibited from displaying obscene pictures in the premises or
surroundings and signs should be displayed at all times at easily visible place outside the premises.
3.7. The licensee shall display a simple sign to be obeyed by employees and customers which are:
No entry
No Smoking
Gambling is prohibited
3.8. A register book containing names, identification card no. /passport no., residential address, photograph and details relating to the employee and masseurs should be provided and constantly updated.
4. Conditions of Employees and Massage Therapists:
4.1. Workers and massage therapists under the age of 18 are not allowed work in a Health Care Center.
4.2. Employees who provide massage therapy must pass an inspection health by a medical officer every six (6) months.
4.3. Workers and massage therapists must wear clean, decent and non eyes flashing uniforms.
4.4. All employees must wear a name tag.
4.5. It is not allowed to cook or bring food inside beauty centers and health care centers except in special places provided.
4.6. Customers under the age of 18 are not allowed to enter the center beauty or health care center.
4.7. Copy of valid passport, health examination certificate, work permit for non-citizens and a copy of the identity card for citizens as well recent photo (passport size) must be included when making license application.
Security aspects:-
i. Licensees need to take into account safety factors in determine the suitability of the premises and the location of the activity venue carried out.
ii. All alterations or additions to business premises must obtain permission from the MBSA Building Department and Fire Department certificate.
iii. Fire prevention equipment such as fire extinguishers shall be adequately prepared and installed in place which is easy to see.
iv. All paths, stairs, entrances and exits must be free from any obstacle.
v. Have a complete and enough emergency treatment medicine chest.
vi. The entire premises must be clean and free from insects and rodents/animals.
vii. Every application is subject to a deposit as a guarantee to the applicant to always comply with the rules and the terms of the granting of the license.
viii. The deposit/security deposit will be confiscated when the applicant violate the terms and provisions of the enactment. Deposit payment rate/collateral money:-
Health Care Center : RM2,000.00
4.8. The premises are allowed to operate from 10.00 am until 12.00 night only.
4.9. A license may be revoked by the Licensing Officer at any time time through a written notice if the licensee is found to be in breach of any condition or restriction and/or violation provisions of law or any regulation.
4.10. Any conditions in these guidelines may be amended by the Council from time to time
from time to time as deemed appropriate.
7. Conditions of Licensing Department (Health Care Center)
